What should you know about perineal carbuncle?

A carbuncle may look like several swollen bumps joined beneath red, tender skin. It can contain pus, feel warm and cause throbbing pain. The perineum has delicate skin and lies close to the anus and genitals, so infection can spread through nearby tissue or be confused with a cyst, abscess or another condition.

A clinician checks the size, depth and location, and may assess whether there are multiple sites or signs of a wider infection. The appearance does not show which bacteria are present, so do not rely on squeezing it or using leftover medicine.

Arrange prompt medical assessment. Keep the area clean, cover any drainage with a clean dressing, wash hands after contact and wear loose clothing. A clinician may recommend antibiotics for skin infection, abscess drainage, or both, depending on the findings. If drainage is advised, it should be performed with appropriate pain control and wound-care instructions.

Do not pierce, cut or squeeze the carbuncle. Do not share towels or razors, and tell the clinician about diabetes, immune problems, recurrent boils and medicine allergies.

Seek urgent help if pain or swelling escalates quickly, redness spreads, the skin becomes dark, fever develops, or you feel weak, confused or faint. Emergency review is also needed for difficulty urinating, severe genital or anal pain, heavy drainage or a wound that rapidly worsens.

Ask whether the lump needs drainage, whether a sample should be tested, which wound-care steps to follow, how to take any antibiotic, and when the area should be rechecked. Ask how to reduce friction and what changes require urgent review.
