What should you know about palmoplantar lichen planus?

The skin may develop firm or flat-topped bumps, rough patches, cracks, or a darker or purplish tone. Thick palm and sole skin can make the changes look different from lichen planus elsewhere on the body. Itching may be persistent, while pressure from walking or gripping can add soreness.

A clinician may examine your hands, feet, mouth, nails, and other skin because lichen planus can involve more than one area. Similar-looking conditions include eczema, psoriasis, and fungal infection, so an examination is useful before treatment.

Arrange a dermatology review if the changes persist, spread, interfere with walking or hand use, or keep returning. Until then, use a gentle cleanser, moisturise thickly after washing, and avoid picking or rubbing the affected skin. Comfortable footwear and protective gloves can reduce friction.

Seek prompt medical advice if cracks become increasingly painful, warm, swollen, or drain fluid, or if walking becomes difficult. Arrange urgent care for rapidly spreading redness, severe swelling, fever, or feeling unwell, as these can signal infection or a more serious reaction.
