Pain Under the Tip of the Thumb Nail

The nail tip can hurt after catching it on an object, trimming too closely, biting the nail, or repeated pressure from tools. Check for a torn edge, a small splinter, redness beside the nail, or bruising beneath it. If the discomfort is deeper at the thumb base and appears with gripping or wrist movement, a tendon problem such as De Quervain's Tenosynovitis may be assessed.

Pain with warmth, swelling, or discharge needs different attention from isolated tenderness after pressure. Avoid assuming the nail colour alone identifies the cause; an examination can distinguish nail-bed injury from skin, joint, or tendon pain.

Stop the activity that presses on the nail and keep the fingertip protected with a loose clean dressing if the skin is broken. Wash gently with soap and water, do not cut into the nail corners, and use a cool wrapped pack for short periods if swollen. Avoid picking at a loose nail or trying to drain a dark area yourself.

Seek prompt assessment for a crush injury, severe or increasing pain, a deep wound, reduced thumb movement, numbness, or a nail that is separating. Get urgent care if redness spreads, pus appears, fever develops, or the fingertip becomes cold or changes to an unusual colour. Have persistent pain or a new dark area checked if it does not improve or move outward as the nail grows.

Ask what tissue is tender, whether the nail bed needs treatment, how to dress or protect the fingertip, and which activities to pause. Tell the clinician about recent trauma, trimming, work pressure, swelling, and any colour change.
