Why extension may hurt

Leaning backwards narrows and loads parts of the lower spine. Facet joint syndrome, lumbar spondylosis, or a muscle strain may therefore hurt during extension or prolonged standing. Sacroiliac joint dysfunction can cause one-sided pain near the pelvis. A herniated disc may cause back pain with leg pain, tingling, or weakness, while spinal stenosis more often causes leg symptoms with standing or walking. What to do

Avoid repeatedly forcing the painful arch and modify activities that require prolonged extension. Keep walking and change positions regularly if these are comfortable. Use a short period of relative rest after a flare, then resume ordinary activity gradually. Arrange assessment if pain persists, restricts work or sleep, or repeatedly returns. Physiotherapy may use trunk and hip strengthening, movement retraining, and a graded return plan matched to the examination.

When to seek urgent care

Get emergency help for new loss of bladder or bowel control, numbness around the genitals or inner thighs, or severe weakness in both legs. Seek urgent assessment after major trauma, with fever and severe back pain, unexplained weight loss, or rapidly worsening symptoms. Prompt review is needed when leg pain, numbness, or weakness is progressing.
