Understanding tibial X-ray findings

The tibia is the larger weight-bearing bone of the lower leg. When infection affects it, an X-ray may show reduced or patchy density, an irregular outer surface, thickened bone, or new bone formed around an older infected area. A cavity or separated dense fragment can suggest damaged bone. Swelling around the tibia may be seen, but the image does not fully show muscles, skin or small fluid collections.

A tibial X-ray can look normal early in the illness. The clinician combines the image with tenderness, warmth, swelling, a draining wound, fever, previous fracture or surgery, and blood-test results. More detailed imaging or a sample may be recommended if the diagnosis remains uncertain.

Do not put extra weight through a painful lower leg until a clinician advises you. Arrange follow-up to review the actual images and decide whether more tests are needed. Keep any wound protected as instructed, do not remove a scab or drain fluid yourself, and mention metalwork, previous operations, diabetes or circulation problems.
