Osteomyelitis and an Open Wound: What to Know

An open wound can break the skin barrier and let germs reach muscle or bone, particularly after a deep injury, operation, pressure injury, or ulcer. Warning features include increasing pain, warmth, swelling, pus, odour, exposed bone, or a wound that remains open or repeatedly drains. Fever may be absent. A clinician will assess the wound, circulation, sensation, and depth, then use blood tests and imaging when needed. A tissue or bone sample may guide antimicrobial choices.

Arrange prompt assessment rather than covering a worsening wound and waiting. Keep the dressing clean and follow the wound-care instructions; wash your hands before and after touching it. Do not scrape the wound, squeeze drainage, apply household substances, or use leftover antibiotics. Protect the area from pressure and follow advice about bathing, walking, and footwear. Treatment can involve prescribed antimicrobial medicine, professional cleaning, drainage, and removal of unhealthy tissue. Reconstruction is planned only when infection and the wound environment are controlled.

Seek same-day care for spreading redness, increasing warmth or swelling, pus, bad odour, exposed bone, worsening pain, or a wound that suddenly opens. Go to emergency care for fever with confusion, faintness, rapidly spreading skin changes, severe weakness, a cold or pale limb, or loss of feeling or movement.

How deep is the wound, and is bone exposed? Do I need imaging or a tissue sample? How should the dressing be changed? How much pressure or walking is safe? What signs mean I should return urgently?
