What causes osteomyelitis?
The infection may start in another part of the body and travel in the blood to bone. It can also extend from an infected wound, ulcer, joint, or soft tissue next to the bone. An open fracture, puncture wound, or surgery can give germs a direct route into bone.
How a bone infection develops
Bone infection can cause inflammation inside bone, where swelling may reduce local blood flow and make healing more difficult. Risk is higher when a wound is deep or slow to heal, when metalwork or another implant is present, or when circulation and immune responses are impaired. Diabetes, reduced sensation in the feet, kidney disease, and medicines that suppress immunity can affect how an infection begins or progresses, but osteomyelitis can also occur without these factors.
A clinician may assess the wound and the affected limb, then use blood tests and imaging. A sample from bone or infected tissue can help identify the germ and guide treatment.
What to do if it may be present
Arrange a medical assessment promptly for persistent bone or wound pain, swelling, warmth, drainage, or a wound that is not healing. Keep the area protected and follow wound-care instructions. Do not squeeze a draining wound or stop prescribed antimicrobial medicine early. Treatment may involve targeted antibiotics or antifungal medicine, wound care, removal of infected tissue, or repair of damaged bone. Bone grafting may be considered when infection or injury has left a gap that needs reconstruction.
When to seek urgent care
Seek urgent medical help if fever or chills occur with a painful or draining wound, redness is spreading, swelling is rapidly worsening, the limb becomes difficult to move, or you feel faint, confused, or unusually weak. After a fracture or operation, new drainage, wound separation, or worsening pain also needs prompt review. People with diabetes, poor circulation, or reduced sensation should seek care early because a foot wound may become serious before pain is obvious.
