How infection reaches bone
Bacteria may reach bone through a wound, surgery, an open fracture, or spread through the bloodstream from another infection. Skin organisms such as Staphylococcus aureus are common causes, but the responsible organism can vary with the body site, injury, previous treatment, and a person’s immune health. Some infections involve more than one organism.
Fungi are less common and are considered more carefully when immune defences are reduced, exposure is unusual, or the illness does not respond as expected to initial treatment. Symptoms can include persistent bone pain, warmth, swelling, drainage, fever, or reduced function, but symptoms alone cannot identify the organism.
How it is investigated and treated
Evaluation may include an examination, blood tests, and imaging. When feasible, a sample from the infected area is tested so treatment can be directed at the organism rather than guessed from symptoms alone. Treatment may involve antimicrobial medicine, wound care, and a procedure to drain infection or remove unhealthy tissue. Bone reconstruction is a separate consideration when infection or injury has left a structural defect.
When to seek care
Arrange prompt medical assessment for worsening pain with fever, a draining wound near bone, spreading redness, or swelling after surgery or an open injury. Seek urgent care if you feel faint, confused, severely unwell, or cannot use the affected limb. People with diabetes, poor circulation, or reduced immunity should report suspicious wounds early.
