What should you know about onycholysis toe?

Short answer
Toenail onycholysis needs gentle trimming, dry protection, and assessment when the separation is persistent or changing.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

What happens to a lifted toenail
With onycholysis, the toenail plate is no longer firmly attached to part of the nail bed. The gap may collect debris and can make the nail look pale, yellow, or uneven. Toe injuries, running or other repeated pressure, tight shoes, prolonged dampness, Nail Fungus, and Nail Psoriasis are possible contributors. Several causes can look alike without an examination.
Caring for the toe
Wear shoes that leave space above and around the toes, and choose clean, dry socks. After washing, dry around and beneath the free edge without forcing anything into the gap. Trim only loose nail that catches, keep tools clean, and avoid cutting into the attached area. Do not cover the nail with acrylics or polish while the cause is unclear. A clinician can tailor treatment after checking the nail and skin.
When the toe needs review
Seek medical advice if the nail continues to lift, becomes thick or crumbly, or separates after an injury without improving. Prompt review is needed for pus, worsening pain, swelling, warmth, spreading redness, fever, or a dark mark that does not grow out. If you have diabetes or circulation problems, arrange advice promptly for any toe wound or nail injury.
What to ask
Ask whether footwear, trauma, infection, or a skin condition is contributing, whether a nail sample is useful, and how to protect the toe during growth. If the loose portion is extensive or repeatedly painful, ask whether Nail Surgery is an option.

Questions people ask
Can I exercise with a lifted toenail?
Activity may be possible if the toe is comfortable and protected, but stop if footwear causes rubbing, bleeding, or increasing pain.
How should I clean under the nail?
Let clean water flow over the area and dry it gently; avoid scraping beneath the nail because this can injure the nail bed.
