What should you know about old burn scar itching?

Short answer
Old burn scar itching can come from dry, tight scar tissue or altered nerves, but a new rash, wound, or changing scar should be assessed.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

What old burn scar itching can mean
A scar may feel itchy, tight, numb, tingly, or unusually sensitive even after the burn has healed. Scar tissue does not behave exactly like uninjured skin: it may hold less moisture, stretch differently, and transmit nerve signals in an altered way. Friction from clothing, heat, sweating, or scratching can make the sensation more noticeable.
Itching can also come from a separate skin problem affecting or surrounding the scar, such as irritation from a product, eczema, psoriasis, hives, or an infection. The scar itself does not prove that one of these conditions is present. Look for clues such as scaling, spreading redness, blisters, oozing, crusting, or itch elsewhere on the body. A clinician can distinguish scar-related sensitivity from a new skin condition by examining the area.
Ways to calm the itch
Keep the area moisturised with a plain, fragrance-free emollient, especially after washing. Use lukewarm water, pat the skin dry, and choose loose, breathable clothing if fabric rubs the scar. A cool compress wrapped in cloth may temporarily settle the urge to scratch. Keep fingernails short and press or gently tap the area instead of scratching, which can break fragile skin.
Avoid starting steroid creams, numbing products, essential oils, or abrasive scar treatments on your own when the skin is cracked, infected, or changing. If itching persists, disrupts sleep, or limits movement because the scar feels tight, arrange a dermatology or primary-care review. Treatment may depend on whether the main issue is dryness, inflammation, infection, or nerve sensitivity.
When to seek medical care
Seek prompt medical advice if the scar becomes increasingly red, warm, swollen, painful, or starts draining fluid; if it develops an open area, bleeding, a persistent crust, or a lump; or if redness is spreading beyond the scar. A fever or feeling unwell alongside skin changes also needs prompt assessment. Arrange a routine appointment for itch that keeps returning, becomes more intense, or comes with a new rash, colour change, thickening, or reduced movement.
Questions for your doctor
Ask whether the itch appears to be caused by scar dryness, inflammation, infection, or altered nerve sensation. You can also ask which moisturiser is suitable, whether a prescribed cream is appropriate for the scar's current condition, and whether the scar's tightness or texture would benefit from specialist treatment. Bring a list of products that touch the area and mention when the itching began, what triggers it, and whether it affects sleep or movement.

Questions people ask
Can an old burn scar start itching years later?
Yes. Dryness, friction, heat, changes in nerve sensation, or a separate rash can make an established scar itchy later on. New or worsening skin changes should be examined.
Should I scratch an itchy burn scar?
Try pressing, tapping, or using a cool cloth instead. Scratching can damage the surface and allow irritation or infection to develop, especially if the scar is fragile.
Is itching alone a sign that a burn scar is infected?
Itching alone does not establish infection. Increasing warmth, swelling, pain, spreading redness, drainage, an open area, or fever are more concerning features.
