The ring supplies hormones that keep the uterine lining thinner and control the usual cycle pattern. The bleeding during the ring-free interval is withdrawal bleeding rather than a necessary monthly period. Some people have spotting between bleeds, while others notice little or no bleeding. Changes are more likely during the first few cycles, after missed or late ring changes, or when the ring is used continuously to skip the ring-free interval.

Keep using the ring on schedule unless a clinician tells you to stop. If the ring was used correctly and there are no pregnancy symptoms, one absent withdrawal bleed may occur without meaning something is wrong. Take a pregnancy test and contact a clinician if the ring was late, out for too long, or if bleeding is absent again. Do not insert extra rings to trigger bleeding. Record spotting and bleeding dates so your clinician can see the pattern.

Seek urgent care for very heavy bleeding with dizziness, severe pelvic pain, fainting, or a positive pregnancy test with one-sided pain. Contact a clinician for bleeding after sex, persistent pelvic pain, unusual discharge, or a new pattern that continues and concerns you. Sudden chest pain, breathlessness, or a painful swollen leg also needs urgent assessment.
