What should you know about normal imperforate hymen?

An imperforate hymen is a congenital tissue membrane that fully covers the vaginal opening and can block menstrual flow. It is present from birth and may be noticed in infancy, during puberty, or when menstrual bleeding is expected but does not appear.

As menstrual fluid collects behind the membrane, some adolescents develop increasing lower abdominal or pelvic pain, pressure, backache, or difficulty passing urine. A bulging membrane may be visible at the vaginal opening, but only a clinician should assess it. Symptoms can resemble other causes of delayed puberty or periods stopped, so the name should not be assumed from pain alone.

Assessment usually includes a private conversation and a careful examination. A Pelvic Ultrasound may help show the uterus, vagina, and any retained fluid when the clinician needs more information. The condition is different from a variation in hymenal shape that still allows menstrual flow.

Arrange an appointment with a gynaecology or adolescent-health clinician if periods have not started alongside other expected puberty changes, or if recurring pelvic pain and pressure are present. Bring a trusted support person if wanted and write down pain timing, urinary symptoms, and any bleeding.

Treatment is a small procedure that creates an opening so trapped fluid can drain and future menstrual flow can pass. The clinician explains anaesthesia, wound care, activity guidance, and follow-up. Do not try to open the tissue yourself or insert objects, because this can cause injury and infection.

Seek urgent assessment for severe or rapidly worsening pelvic pain, inability to pass urine, fainting, fever, repeated vomiting, or a swollen abdomen. These signs can indicate pressure or infection and need timely review. New bleeding, a painful lump, or symptoms that interfere with walking, sleep, or daily activities also deserve prompt clinical attention.

Ask whether the examination supports an imperforate hymen, whether imaging is needed, and what other causes of absent periods should be considered.
