What should you know about nonbacterial osteomyelitis?

Short answer

Nonbacterial osteomyelitis is inflammation inside bone that can cause pain, swelling, and reduced movement without a typical bacterial infection.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

At a glance

Also called
Chronic nonbacterial osteomyelitis may be used for some patterns of this condition.
Key distinction
Inflammation is present, but a typical bacterial infection is not the assumed cause.

What it means

The condition may affect one bone or several areas, including the spine, pelvis, arms, or legs. Symptoms can come and go, and discomfort may be accompanied by tenderness, warmth, or stiffness near an affected area. Because these features can resemble infection, injury, arthritis, or another inflammatory illness, assessment usually combines your history, examination, blood tests, and imaging. A scan or biopsy may be considered when the diagnosis remains uncertain. Antibiotics are not automatically useful when bacteria are not the cause.

What to do next

Arrange a medical review if persistent bone pain, swelling, or stiffness is limiting sleep, walking, school, work, or exercise. Keep a record of where symptoms occur, whether they fluctuate, and any fever, skin changes, bowel symptoms, or joint complaints. Follow the treatment plan provided; it may include activity adjustment, pain relief, physiotherapy, or medicines aimed at reducing inflammation. Protect painful areas from high-impact loading until a clinician has assessed them, while maintaining gentle movement if advised. Bone grafting is a separate surgical treatment used for selected bone problems, not a routine answer for every inflammatory bone symptom.

When to seek care

Seek urgent medical help for severe or rapidly worsening pain, a new inability to use a limb, marked swelling, a hot red area with fever, confusion, or feeling faint. Prompt assessment is also needed for new weakness, numbness, loss of bladder or bowel control, or severe back pain after an injury. These warning signs can reflect infection, a fracture, nerve pressure, or another problem requiring immediate attention.

Questions for your doctor

Which bone or bones appear involved, and what findings support the diagnosis? Do I need blood tests, an MRI, or a biopsy? Which activities should I pause, and what movement is appropriate? How will pain and inflammation be monitored? What changes should prompt an earlier review?

Questions people ask

Is nonbacterial osteomyelitis the same as a bone infection?

No. Its symptoms may resemble a bone infection, but the underlying process is inflammation rather than a typical bacterial infection.

Can the symptoms affect more than one place?

Yes. Some people have symptoms in more than one bone or in nearby joints, so tell your clinician about every painful or swollen area.
