Night-time back spasms are sudden involuntary tightening of back muscles that can interrupt sleep and restrict movement.

What it means

A spasm can feel like a hard knot, cramp or gripping pain. It may follow unusual lifting, prolonged sitting, a change in activity or an irritated joint or disc. Muscle strain, facet joint syndrome, herniated disc and non-specific low back pain can all be considered, but a night-time spasm does not identify one cause by itself.

Note when the spasm starts, its location, how long it lasts, and whether pain travels into a leg. Numbness, tingling or weakness changes the urgency of assessment.

What you can do

Change position slowly, use comfortable support under or between the knees, and try gentle walking once the contraction eases. A warm pack may relax tight muscles; protect your skin and stop if symptoms worsen. Avoid forceful stretching or sudden twisting during an active spasm.

If episodes recur, a physiotherapy assessment can examine movement, strength and lifting habits. Myofascial release or dry needling may be discussed when suitable, alongside exercise and gradual return to normal activity. Do not start these treatments without an assessment if symptoms include neurological changes.

When to seek care

Arrange review when spasms repeatedly disturb sleep, persist despite simple measures, or follow an injury. Seek urgent care for new bladder or bowel difficulty, numbness around the groin, progressive leg weakness, fever with severe back pain, or pain after major trauma.

Ask what examination findings may explain the spasms, whether nerve irritation is present, and which movements are safe. Ask for a staged plan for sleep, work, lifting and recurrence prevention.
