Short answer

Vulvar neurodermatitis is an itch-scratch cycle that causes irritated, thickened vulvar skin and needs assessment to exclude other causes.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

At a glance

Core mechanism
Itch followed by rubbing and scratching
Typical change
Thickened, irritated vulvar skin
Treatment principle
Break the itch-scratch cycle and identify triggers

What it means

Neurodermatitis begins with itch or irritation. Scratching and rubbing then damage the surface, making the skin thicker, more sensitive, and itchier. On the vulva, friction from clothing, moisture, fragranced products, shaving, discharge, eczema, or another skin condition may help maintain the cycle. The affected skin can feel rough or leathery and may show colour change or small scratch marks.

Because infections, contact dermatitis, atopic dermatitis, lichen sclerosus, and other conditions can look similar, an examination is useful when symptoms are new, persistent, painful, or changing. Vulvar symptoms should not be self-diagnosed from itch alone.

What to do at home and with treatment

Wash gently with lukewarm water or a fragrance-free cleanser recommended for sensitive skin. Pat dry, choose loose breathable underwear, avoid scented wipes and sprays, and keep nails short to reduce skin injury during sleep. A plain protective emollient may reduce friction if your clinician says it is suitable.

Treatment may include a prescribed topical corticosteroid used in a specific amount and schedule, followed by review. Do not apply steroid, antifungal, or anaesthetic products repeatedly without guidance because the vulvar skin is sensitive and the wrong product can mask another diagnosis. Intralesional steroid injection is reserved for selected cases assessed by a specialist.

When to seek care

Arrange prompt medical review for bleeding, open sores, blisters, severe pain, a new lump, marked colour change, urinary difficulty, or discharge with a strong odour. Seek urgent help for rapidly spreading swelling, fever with severe pain, or difficulty passing urine. Any persistent vulvar change deserves examination rather than prolonged scratching and self-treatment.

Questions for your doctor

Ask what may have started the itch, whether infection or another vulvar condition needs testing, which topical corticosteroid schedule is appropriate, and how to protect the skin between applications. Ask when follow-up is needed if thickening or itch continues.

Questions people ask

Is vulvar neurodermatitis caused by stress alone?

Stress can intensify itch or scratching, but friction, products, eczema, infection, and other skin conditions may also contribute.

Should I use a steroid cream on vulvar skin?

Only use a steroid prescribed or recommended for that area, with the exact schedule provided by a clinician.
