What should you know about neoplastic spinal cord compression?

Cancer can weaken a spinal bone, grow in or around the spine, or cause swelling that narrows the space around the spinal cord. Pressure may affect the cord or the nerve roots leaving it. Back or neck pain can be severe, persistent, worse when lying down, or aggravated by coughing or movement. Pain may travel around the chest or down a limb. Weakness, numbness, unsteady walking, and changes in bladder or bowel control are especially concerning.

Doctors may arrange urgent examination and magnetic resonance imaging. Treatment depends on the scan, cancer type, spinal stability, and your overall health. It may include steroids, radiotherapy, surgery to remove or stabilise tumour, pain relief, and treatment directed at the underlying cancer. The aim is to relieve pressure and preserve function.

If this condition is suspected, contact your oncology team or emergency service immediately and explain that you have new neurological symptoms or severe spinal pain. Avoid delaying assessment for a routine appointment. Keep a list of medicines and cancer treatments with you, and follow instructions about movement until the spine has been assessed. Do not drive yourself if weakness, severe pain, or difficulty walking is present.

Seek emergency care now for new leg or arm weakness, numbness that is spreading, difficulty walking, loss of bladder or bowel control, inability to pass urine, or severe new back or neck pain in someone with cancer. Rapid worsening can cause lasting nerve damage. Call emergency services if you cannot walk safely, have collapsed, or develop severe breathing difficulty.
