What should you know about neonatal osteomyelitis?

Newborns may not show the familiar signs of infection. A baby can have fever or an unusually low temperature, feed poorly, seem very sleepy or irritable, or move one arm or leg less than usual. Swelling, warmth, or tenderness around a limb may be present, but the skin can look normal. The infection can involve a growing bone and nearby joints, so clinicians assess movement, circulation, and the baby's overall condition.

Contact the baby's neonatal or paediatric team promptly if these changes appear. Assessment may include an examination, blood tests, and imaging chosen for the baby's age and symptoms. Treatment commonly involves antibiotics, initially through a vein, with the medicine and duration adjusted to the suspected organism and response. Do not massage a swollen limb or delay care while observing symptoms at home. A hospital team may involve neonatology, paediatrics, infectious diseases, and orthopaedics. Bone Grafting is a separate procedure used only in selected bone problems, not a routine first step for a newborn's suspected infection.

Seek emergency help for a newborn who is difficult to wake, has breathing difficulty, is blue or unusually pale, refuses several feeds, has repeated vomiting, has a seizure, or feels markedly hot or cold. Get same-day medical advice for reduced limb movement, new swelling, persistent crying when a limb is handled, or a change in feeding or alertness. Newborns can become unwell quickly, and absence of fever does not rule out a serious infection.
