Nappy rash on the scrotum: what to know

Delicate scrotal skin may become red or sore inside a damp nappy, often alongside groin or buttock irritation. Yeast, eczema, and contact reactions are other possible skin causes. Sudden one-sided swelling, severe pain, a firm change, or a testicle positioned differently is not typical simple nappy rash and needs urgent assessment.

Change nappies promptly, rinse with lukewarm water, and pat the scrotal and groin skin dry without rubbing or stretching it. Avoid talcum powder, antiseptics, scented products, and medicated creams unless prescribed. A bland protective emollient may help external irritated skin; see Emollient Therapy. If folds are involved or the rash persists, a clinician may assess whether Topical Antifungal Treatment is appropriate.

Seek urgent care for sudden severe scrotal pain, rapid swelling, blue or dark colour, vomiting, fever, or a testicle that appears higher than usual. Get prompt advice for pus, blisters, open skin, spreading warmth, or a child who is difficult to settle. Mild surface redness can be reviewed if it persists or returns.
