Muscle strain while sleeping: what to know

Short answer
A muscle strain may hurt while sleeping when a position keeps the muscle shortened, stretched, compressed, or repeatedly loaded during the night.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Night trigger
- Compression, prolonged stretch, or an awkward sleeping position
- Comfort measure
- Support the arm, knees, or affected area without forcing it
- Review sooner
- Constant night pain, neurological symptoms, fever, or injury
What it may mean
Sleep does not usually create an injury without a preceding load, but an awkward position can irritate a muscle that was already tired or mildly strained. You may notice stiffness on waking, pain when turning, or tenderness after sleeping on the affected side. Night pain can have other causes, especially when it is constant, severe, or unrelated to movement.
What you can do
Try the position that leaves the area most relaxed: side-lying with a pillow supporting the top arm or between the knees, or back-lying with support under the knees. Avoid sleeping directly on a tender area. Gentle movement after waking can reduce stiffness; use cold or warmth according to comfort. During the day, avoid the triggering load but do not stay still for long periods. See Muscle Strain and Muscle Cramps for related guidance.
When to seek care
Seek urgent help for night pain with breathing difficulty, chest pressure, faintness, new weakness, loss of bladder or bowel control, or severe pain after injury. Arrange medical review for fever, unexplained night sweats, progressive pain, numbness, or pain that repeatedly wakes you despite position changes. A clinician can examine the area and check whether the problem is muscular.
Questions for your doctor
Which sleep position and pillow support suit my symptoms? Does the pain behave like a muscle strain? What waking or night-time symptoms should prompt urgent care? How should I pace daytime activity while healing?

Questions people ask
Can a bad sleeping position cause a pulled muscle?
It can irritate an already overloaded muscle or cause short-lived stiffness, while a more significant strain usually follows a distinct load or sudden movement.
Should I use heat before bed?
Gentle warmth may relax stiffness for some people. Protect the skin, avoid excessive heat, and stop if symptoms worsen.
