Morphea on the legs: what to know

Patches on the thighs, shins or calves may not all be at the same stage. One can have an active coloured edge while another is flatter and pale or darker after inflammation. Several areas can make it harder to judge the overall pattern, so examination of the full skin surface is useful. If a patch crosses a knee or ankle, the clinician also checks flexibility and signs of deeper tethering.

Keep a simple record of new areas, changes in colour, firmness, itch and movement. Use bland moisturiser for dryness and protect exposed skin from sun so colour differences are less noticeable. Seek an assessment before trying cosmetic procedures or strong topical products over uncertain patches. Treatment may involve local prescription medicine, Narrowband UVB Phototherapy when appropriate, or therapy for deeper active disease. Topical Calcineurin Inhibitors are among the local options a specialist may discuss.

Seek earlier review when new patches appear quickly, existing areas become hard or painful, or bending a knee, moving an ankle or walking becomes difficult. A red-hot expanding area with fever, sudden one-sided swelling, marked weakness or severe pain needs urgent medical attention. These warning features should be judged on their own and should not be explained away as morphea without examination.

Ask whether every patch has the same activity, whether any area is deep, and whether joints need a movement assessment. Ask how to photograph or measure change, when follow-up is needed, and whether a biopsy or another specialist would add useful information.
