What should you know about modafinil rash?

Modafinil can be followed by a skin eruption, but not every rash during treatment is caused by the medicine. A drug rash may be flat or raised, itchy, widespread or localised, and can appear after treatment begins or after a dose change. Other possibilities include Contact Dermatitis, Atopic Dermatitis, Heat Rash, Ringworm or Scabies. The medicine history, distribution, timing and associated symptoms help a clinician separate these causes. Do not assume a mild-looking rash will remain mild.

Contact the prescriber or pharmacist as soon as a new rash appears and give the medicine name, dose, start date, recent changes and all other medicines or supplements. Take photographs and avoid applying unfamiliar creams that could obscure the pattern. Do not restart a stopped medicine or take another person’s treatment. The prescriber will balance the reason for modafinil against the skin findings and tell you whether it should be withheld or replaced.

Seek emergency care immediately for facial or tongue swelling, breathing difficulty, faintness, fever with a rapidly spreading rash, skin tenderness, blisters, peeling, or sores in the mouth, eyes or genitals. These features can indicate a serious medicine reaction. Go urgently as well if you feel profoundly unwell or cannot drink. Bring the medicine packaging and a list of everything taken recently.

Ask whether the appearance fits Drug Rash, whether modafinil should be paused, and which alternative medicines are safe. Clarify which symptoms require emergency help, whether blood tests or skin review are needed, and how this reaction should be recorded for future prescribing. Mention any previous reaction to medicines, even if it involved a different body system.
