What should you know about Imodium and black stool?

Imodium, also called loperamide, slows bowel movement and can relieve some short-term diarrhoea. It does not normally turn stool black. Black stool may follow iron, bismuth-containing products, or certain foods, but stool that is tar-like, sticky, unusually foul-smelling, or accompanied by weakness can reflect digested blood from the stomach or upper intestine. Possible causes include gastritis, a peptic ulcer, or Helicobacter pylori infection. Colour alone cannot identify the cause, so a clinician may ask about medicines, examine you, and arrange blood or stool tests.

Do not use extra Imodium to suppress diarrhoea until a clinician has considered the black stool, particularly if you also have pain, fever, blood, or abdominal swelling. Make a list of all medicines, including aspirin, anti-inflammatory painkillers, iron, and stomach remedies. Note when the colour changed and whether stools are sticky. Avoid anti-inflammatory painkillers unless a healthcare professional says they are suitable, and arrange prompt medical advice even if the stool appears only once.

Go to emergency care for vomiting blood or material like coffee grounds, fainting, severe dizziness, rapid heartbeat, marked weakness, severe abdominal pain, or repeated black stools. Call for urgent help if you become pale, confused, breathless, or unable to stand safely. These signs can accompany significant gastrointestinal bleeding and should not be attributed to Imodium without assessment.
