What should you know about imiquimod for superficial BCC?

Superficial BCC is a type of basal cell carcinoma that is limited mainly to the upper layers of skin. Imiquimod is a cream applied to the affected area; it is not a general moisturiser or a cream for every suspicious mark. The medicine activates immune signals in the treated skin, so redness, scaling, crusting, soreness or weeping can occur as the area responds. These changes can look alarming, but the dermatologist needs to interpret them alongside examination findings. Imiquimod is generally considered only for selected lesions in suitable locations and situations. A biopsy or other clinical assessment may be needed before treatment.

Follow the written plan exactly, including how much cream to use, which skin to cover, how often to apply it and when to wash it off. Apply to clean, dry skin with clean hands, then wash your hands afterwards. Keep the cream away from the eyes, lips, inside of the nose and broken skin unless your clinician gives different instructions. Do not add dressings, cosmetics or other active creams over the area without checking first. Protect the treated site from strong sun and attend the planned review, because the skin can heal while microscopic disease still needs assessment.

Contact your dermatology team if the reaction becomes difficult to tolerate, the area develops spreading warmth or pus, bleeding does not settle with gentle pressure, or you feel unwell. Seek urgent help for swelling around the eyes or face, trouble breathing or a rapidly spreading blistering rash. Do not restart, extend or shorten the course after a break without advice. A lesion that remains, returns or changes after treatment needs examination rather than another unsupervised application.

Ask whether the lesion is confirmed as superficial BCC, why imiquimod fits its site and depth, what reaction is expected, how missed applications should be handled, and when the response will be checked. Also ask what alternatives are available if the lesion does not clear or if the local reaction is too severe.
