Iliopsoas calcific tendinitis: what to know

Short answer
Iliopsoas calcific tendinitis involves calcium deposits irritating a hip-flexor tendon, which can cause groin pain, stiffness, and pain when the hip is moved or loaded.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

What is happening
The iliopsoas tendon connects deep hip-flexor muscles to the upper thigh region. Calcium can collect within or near the tendon and trigger irritation. Symptoms may include deep groin pain, pain with hip flexion, stiffness after sitting, a catching sensation, or difficulty with stairs and longer walks. The term calcific tendinitis is also used for similar calcium-deposit problems in other body regions, but this page concerns the hip rather than shoulder pain or pain when lifting the arm.
Assessment usually combines the symptom pattern with a physical examination. X-ray or another scan may help show a deposit and exclude other causes of groin pain. The size or appearance of a deposit does not by itself explain every symptom.
Treatment options
Reduce movements that repeatedly provoke pain, especially forceful hip flexion, while maintaining comfortable daily movement. A physiotherapist may prescribe gradual mobility and strengthening work once irritability settles. Depending on the findings, a clinician may discuss anti-inflammatory care, a cortisone injection, shockwave therapy, or a procedure such as shoulder arthroscopy only when another body region is actually involved; hip-specific treatment should be selected for the hip diagnosis. Do not try to massage or press deeply on a painful area without advice.
When to seek care
Seek assessment when groin pain persists, limits walking or sleep, or returns with ordinary activity. Prompt care is needed for fever, a hot swollen joint, sudden inability to bear weight, severe pain after injury, or rapidly worsening symptoms. These signs can indicate a problem requiring more urgent evaluation than tendon irritation alone.
Questions for your doctor
Ask how the deposit was identified, whether the hip joint or tendon is the pain source, and what recovery milestones should guide activity. Discuss the expected purpose, timing, and risks of physiotherapy, injection, or shockwave treatment.

Questions people ask
Is calcific tendinitis the same as a muscle tear?
No. Calcific tendinitis involves calcium deposits and tendon irritation, while a tear is a disruption of muscle or tendon fibres.
Can physiotherapy help?
It may help restore comfortable movement and strength when exercises are matched to the current irritability and diagnosis.
