Ichthyosis in infants: practical guidance

Short answer
Ichthyosis in infants is managed with a tailored skin-barrier routine, medical follow-up, and prompt attention to feeding, temperature, or infection concerns.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

Understanding the condition
An infant may have persistent dryness, fine or thick scales, redness, or skin that cracks easily. Ichthyosis describes a group of disorders, and the pattern can change as a child grows. A clinician considers the distribution of scales, whether symptoms were present at birth, family history, and possible triggers before naming a subtype.
The skin barrier helps retain moisture and protect against irritation. When it is weakened, heat loss, discomfort, rubbing, and infection can become concerns. Itching may disturb sleep, while tight skin around the mouth or eyes can affect feeding or comfort.
Daily care
Use the cleanser, bath temperature, and moisturiser recommended for your infant. Apply the emollient soon after bathing while the skin is still slightly damp, using a gentle smoothing motion. Avoid fragrances, rough cloths, vigorous rubbing, and picking flakes. Dress the baby in soft breathable layers and keep the room comfortably cool rather than overheated.
Keep a simple record of products used and changes in scaling, redness, sleep, feeding, and wet nappies. Follow-up allows the plan to be adjusted as the child’s skin and needs change.
When to seek care
Call the infant’s clinician promptly for fever, low temperature, poor feeding, fewer wet nappies, unusual drowsiness, worsening cracks, spreading redness, pus, blisters, or a new unpleasant smell. Seek urgent assessment if breathing is difficult, the baby cannot keep fluids down, or the eyes cannot close comfortably.
Questions for your doctor
Which ichthyosis pattern is suspected? Which products are safe for this age? How should we bathe and moisturise? How can we recognise dehydration or infection? Should the eyes, ears, or mouth be checked?

Questions people ask
Does infant ichthyosis go away after one treatment?
It often needs ongoing care, and the appearance and treatment plan may change as the infant develops.
Can bathing worsen the skin?
Long, hot, fragranced, or abrasive baths can irritate dry skin; follow the bathing routine advised by the child’s clinician.
