IBS and heartburn

Short answer
Heartburn is not a defining symptom of IBS; recurring burning behind the breastbone may reflect reflux or another upper digestive condition.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- IBS hallmark
- IBS is centred on bowel symptoms; heartburn is an upper digestive symptom.
- Track
- Record timing, meals, lying down, medicines and associated chest symptoms.
IBS and heartburn are different patterns
IBS mainly affects bowel symptoms such as abdominal pain, bloating, diarrhoea and constipation. Heartburn is a burning sensation behind the breastbone or rising toward the throat, often caused by stomach contents moving upward. Acid reflux, gastritis, a hiatus hernia and peptic ulcer disease can produce upper digestive symptoms. Having IBS does not explain every digestive sensation, and heartburn may need its own history and treatment plan. Chest discomfort can also have causes outside the digestive system.
What may help
Note when heartburn occurs, including after meals, when lying down, or with particular drinks or foods. Try smaller meals, remain upright after eating, and avoid eating close to bedtime. If weight is affecting reflux, ask for supportive, non-judgemental advice rather than making abrupt dietary changes. A pharmacist or clinician can advise whether an acid-reflux medicine is suitable and how long to use it. Mention all medicines, including pain relievers, because some can irritate the upper digestive tract.
When to seek urgent care
Get emergency help for chest pressure or pain with breathlessness, sweating, faintness, or pain spreading to the arm, back or jaw. Seek prompt medical advice for difficulty swallowing, vomiting blood, black stools, repeated vomiting, unexplained weight loss, or heartburn that is frequent or worsening. These signs should not be assumed to be IBS or treated only with self-care.
Questions for your doctor
Does this pattern fit reflux, gastritis or another cause rather than IBS? Could any of my medicines be contributing? Do I need an examination or further testing? Which lifestyle changes and acid-reflux medicine are appropriate, and when should I return?
