I think I have an overactive bladder: what should I know?

Short answer

An overactive bladder causes sudden urinary urgency, often with frequent trips, but similar symptoms need a clinical assessment.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

At a glance

Core symptom
A sudden, difficult-to-defer urge to urinate
Helpful record
A bladder diary covering drinks, visits, urgency, and leakage
Treatment approach
Bladder training may be part of a personalised plan

What it may mean

Overactive bladder describes urgency that can occur with frequent urination and night-time waking, with or without urine leakage. The bladder muscle may contract before it is full. Urinary infection, constipation, medicines, bladder irritation, pelvic-floor changes, and other conditions can cause similar symptoms, so urgency alone does not establish the diagnosis. A clinician may ask about fluid intake, caffeine, bowel habits, pregnancy, medicines, pain, and leakage. A urine test may be useful.

Keep a short bladder diary: note drinks, toilet visits, urgency, leakage, and night-time waking. [Leaking Urine](/symptoms/leaking-urine/) can occur alongside urgency, but leakage is not required for overactive bladder.

What to do now

Book an appointment if urgency disrupts sleep, work, travel, exercise, or intimacy, or if it keeps returning. Drink regularly without forcing large amounts, reduce caffeine if it worsens urgency, and address constipation if present. Do not severely restrict fluids, because concentrated urine can irritate the bladder. A clinician may recommend [Bladder Training](/treatments/pelvic-floor-and-rehabilitation/bladder-training/) to gradually lengthen intervals and manage urges. If symptoms remain unclear or complex, [Urodynamic Testing](/treatments/diagnostics-and-imaging/urodynamic-testing/) may assess how the bladder and urethra store and release urine.

When to seek care

Seek prompt medical advice for burning urine, fever, new pelvic or back pain, blood in the urine, vomiting, or difficulty passing urine. Seek urgent care for severe pain, confusion, or inability to urinate. New leakage with leg weakness or numbness needs urgent assessment. Otherwise, arrange a routine review when urgency is persistent, worsening, or affecting daily life.

Questions people ask

Does overactive bladder always cause leakage?

No. Urgency and frequent urination can occur without leakage, although some people leak before reaching a toilet.

Can caffeine make urgency worse?

Caffeine can irritate the bladder or increase urine production for some people, so reducing it may help if you notice a clear link.
