I hate letrozole: what should I know?

Letrozole is an aromatase inhibitor used as endocrine treatment for some hormone-sensitive cancers. Commonly reported problems include hot flushes, sweating, joint or muscle aches, stiffness, tiredness, sleep disruption, mood changes, vaginal dryness, and reduced sexual interest. Symptoms can be mild, persistent, or severe enough to interfere with work, movement, or relationships.

The right response depends on your cancer plan, other health conditions, bone health, and the timing of symptoms. Do not assume you must simply endure them, and do not stop treatment without a shared plan.

Keep a symptom diary showing when each problem starts, how long it lasts, what makes it worse, and how it affects sleep or activity. Ask whether exercise, physiotherapy, vaginal moisturisers, sleep support, or a prescribed medicine is suitable for you. Your team may review the dose schedule, consider a monitored interruption, or discuss a different aromatase inhibitor or endocrine treatment.

Use the treatment information supplied for your medicine and mention all supplements, because some products can interact with cancer care. Continue appointments even when side effects are frustrating.

Contact your oncology team promptly for severe pain, a new fall or suspected fracture, marked weakness, distressing mood changes, or symptoms that stop you functioning. Seek urgent help for chest pain, sudden breathlessness, one-sided weakness, facial swelling, or a severe allergic reaction. Do not wait for a routine visit when a symptom feels acute or dangerous.
