Hypopigmentation after a steroid injection

Hypopigmentation means the skin looks lighter than the surrounding area. After a steroid injection, this can happen when corticosteroid affects pigment-producing cells or spreads into the tissue just beneath the skin. It is usually limited to the injection area, but the appearance can be unsettling.

The patch may be flat, lighter, and without pain or itching. It can appear days or weeks after the injection and may be more visible on darker skin. A small hollow or thinning of the fat under the skin can occur alongside the colour change. The skin often slowly blends back towards its previous colour, although the timing varies and some changes can last.

This reaction does not mean the injection has damaged the joint. It is different from redness, warmth, drainage, or worsening tenderness, which can signal infection or another problem. A clinician can examine the area if the diagnosis is uncertain.

Take a clear photograph in good light and note when the colour change began, where the injection was given, and whether the patch is enlarging. Protect the area from strong sun because surrounding skin may darken and make the patch look more obvious. Use gentle skin care and avoid bleaching products or steroid creams unless prescribed.

Arrange a routine review if the patch is spreading, develops a depression, becomes itchy, or remains a concern. Bring the injection details and a list of medicines. Do not arrange another injection into the same area without discussing the skin change with the treating clinician.

Seek prompt medical advice for increasing pain, warmth, swelling, pus, fever, rapidly spreading redness, blistering, or skin breakdown near the injection site. Seek urgent care for severe illness, breathing trouble, or swelling of the face or throat after any injection. These features need assessment rather than observation at home.

Ask whether the colour change fits a steroid-related reaction, whether the underlying skin is thinned, and whether any examination or dermatology review is needed. Ask how to protect the area from sun and whether future injections should use a different depth or site.
