What the sensation can mean

Heartburn happens when stomach contents move upwards and irritate the oesophagus. Acid reflux is a common explanation, while a hiatus hernia can make reflux easier. Gastritis and a peptic ulcer can also cause upper abdominal discomfort, although they are different problems. Hormone replacement therapy may coincide with indigestion in some people, but chest burning should not automatically be attributed to HRT.

What you can do

Notice whether symptoms follow large meals, lying down, alcohol, spicy food, or a newly started medicine. Smaller meals, remaining upright after eating, and raising the head of the bed may help. A pharmacist or clinician can advise whether acid reflux medication is suitable and whether your HRT needs review. Do not stop prescribed treatment without advice.

When to seek care

Seek urgent help for chest pressure, breathlessness, fainting, sweating, or pain spreading to the arm, back, jaw, or neck. Arrange medical assessment for persistent or worsening symptoms, repeated vomiting, trouble swallowing, vomit containing blood, black stools, or unexplained weight loss. These features need assessment rather than self-treatment.
