How long do custom orthotics take to work for hammer toe?

An orthotic can redistribute pressure beneath the foot and help limit forces that push a bent toe against the shoe. It does not usually straighten a fixed deformity. Early sensations may include unfamiliar contact, mild pressure, or tired feet while your muscles and joints adjust. Improvement is better judged by less rubbing, fewer painful steps, or greater tolerance of ordinary shoes than by expecting the toe to change shape.

Begin with short periods of wear and increase time only when the orthotic feels acceptable. Use shoes with enough depth and a roomy toe box; a tight shoe can continue pressing the bent toe even when the insert is well made. Check the skin after wearing it, especially over the toe joints and any hard areas.
