How does shoulder replacement work?

Shoulder replacement removes damaged joint surfaces and fits implants that restore a smoother, more stable shoulder movement. In a conventional replacement, the worn ball is replaced with a metal component and the socket receives a new lining. The surgeon chooses the design and fixation method after assessing bone, tendons, movement and the cause of joint damage.

The shoulder is a ball-and-socket joint. The damaged surfaces are exposed, prepared and replaced with matching components. A metal stem or cap may support the ball, while the socket can receive a smooth polymer liner. Some designs reverse the normal arrangement: the ball is fixed to the socket side and the cup is placed on the upper arm side. A reverse design may be considered when the rotator cuff cannot provide reliable movement. Shoulder osteoarthritis is one reason replacement may be discussed, but the decision is individual.

Before surgery, review medicines, skin or dental infections, home support and the movements you need for work or daily life. Afterward, a sling protects the shoulder while the wound settles and the joint tissues recover. Move the hand, wrist and elbow as instructed, then progress shoulder exercises with physiotherapy. Avoid pushing, pulling and lifting until cleared. Keep follow-up appointments so movement, strength and the implant can be assessed.

Contact the surgical team for increasing redness, drainage, fever, worsening swelling, uncontrolled pain or new numbness. Seek urgent care for chest pain, breathing difficulty, a cold or discoloured hand, sudden deformity or inability to move the arm after a fall. New shoulder pain after recovery also deserves assessment because several tissues can cause it.

Ask whether a conventional or reverse design fits your tendon and bone condition, which materials will be used, and what movement to expect. Discuss sling time, physiotherapy milestones, sleeping positions, driving, work and warning signs after discharge.
