How does fracture casting work?

After examination and imaging, the clinician may gently reposition the bone if needed, then place padding and a rigid outer layer around the limb. The cast holds nearby joints and the fracture area steady while new bone forms. Plaster of Paris and fibreglass cast material have different handling and weight characteristics; the choice depends on the injury, swelling, age, and stage of treatment.

Some fractures need a temporary splint first because swelling can increase after injury. A cast can treat selected ankle, distal radius, growth plate, or scaphoid fractures, but unstable or displaced injuries may need another approach. Follow-up imaging checks position and healing.

Keep the cast dry unless your care team gives specific washing instructions, and never insert objects to scratch beneath it. Elevate the limb as advised, move uncovered fingers or toes gently, and avoid placing weight on the cast until cleared. Check the skin at the edges and attend scheduled reviews. Once the cast is removed, physiotherapy may help restore stiffness, strength, and coordination.

Seek urgent assessment if fingers or toes become blue, pale, cold, numb, or increasingly difficult to move; swelling or pain rises despite elevation; the cast becomes tight, cracked, wet inside, or develops a sharp edge; or there is foul drainage, fever, or worsening skin pain. A new injury to the casted limb also needs prompt advice.

Ask whether you may bear weight, how to protect the cast during sleep and bathing, when repeat imaging is planned, and which symptoms indicate dangerous pressure. Confirm the expected cast duration for your fracture and when movement or physiotherapy can begin after removal.
