How does bone marrow biopsy work?

Short answer
A bone marrow biopsy works by collecting marrow fluid and a small bone core for laboratory examination.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Samples
- Marrow fluid and bone core
- Common site
- Back of the pelvic bone
What the sample shows
The clinician usually numbs an area over the back of the pelvic bone, then uses a hollow needle to draw liquid marrow and remove a narrow core. The fluid shows individual cells, while the core shows how cells are arranged inside the marrow. A laboratory can assess blood-forming cells, abnormal cells, and changes linked with conditions such as leukaemia. Flow cytometry may classify cells by the markers on their surface.
Pressure or brief discomfort can happen during aspiration, and soreness around the site may continue for a short time. The test is designed to answer a specific clinical question; results may take longer when several laboratory examinations are needed.
Before and after the test
Tell the team about blood-thinning medicines, bleeding conditions, allergies, pregnancy, and whether you are anxious about the procedure. Arrange transport if sedation is planned. Keep the dressing in place as instructed, avoid strenuous activity for the advised period, and use only recommended pain relief. Ask how and when the results will be shared.
When to seek care
Contact the clinical team for persistent bleeding, increasing pain, spreading redness, drainage, fever, or swelling at the site. Seek urgent help for fainting, severe weakness, or trouble breathing after the procedure.
Questions for your doctor
Ask what the biopsy is looking for, whether sedation is available, which medicines need review, how soreness will be managed, and which results may change the next step.
Questions people ask
Is bone marrow biopsy the same as a blood test?
No. A blood test examines circulating blood, while this procedure examines the blood-forming tissue inside bone.
Why might flow cytometry be used?
It helps identify and classify cells by their surface markers, supporting assessment of abnormal blood or marrow cells.
