How does azelaic acid treatment work?

Applied to the skin, azelaic acid has anti-inflammatory and keratinising effects. It helps reduce the build-up of cells that can block pores, while its effect on pigment-forming cells can gradually lessen brown marks left after spots or irritation. That makes it useful for some acne-related marks and post-inflammatory hyperpigmentation.

It can also calm some features of rosacea, including inflammatory bumps and redness, although it is not suitable for every rosacea pattern. It works gradually because the skin needs time to shed pigmented cells and settle inflammation. The product may be a gel, cream, or foam depending on the formulation and prescription.

Use the amount and schedule advised for your product, applying it to clean, dry skin unless your clinician gives different instructions. Start cautiously if your skin is sensitive, and use a plain moisturiser to reduce dryness or stinging. Keep it away from the eyes, lips, and broken skin. Introduce other active products one at a time so irritation can be identified. Daily sun protection helps prevent dark marks from deepening while they improve. Do not apply extra product to speed the process; more irritation can worsen discolouration.

Mild tingling, dryness, or temporary irritation can occur, but contact your prescriber if burning is intense, swelling develops, the skin blisters, or a rash spreads beyond the treated area. Stop and seek urgent help for facial swelling, wheezing, trouble breathing, or widespread hives. If redness, spots, or pigmentation keep worsening despite careful use, the diagnosis or routine may need review.
