How sciatica can feel

Sciatica involves irritation or compression of a nerve root in the lower spine. The resulting symptoms may start in the lower back or buttock and follow the back or side of the thigh and leg. Coughing, sneezing, sitting, bending, or particular positions can intensify symptoms for some people.

Not every buttock ache is nerve pain. Muscle, hip, and sacroiliac problems can produce nearby discomfort. Numbness, pins and needles, burning, or weakness make nerve involvement more relevant, but a clinician still needs the complete history and examination.

Avoid prolonged bed rest and try gentle, regular movement that does not cause a marked increase in leg symptoms. Alternate positions, use short walks if comfortable, and temporarily reduce lifting or bending tasks that clearly worsen the pain.

A clinician or physiotherapist can help you find positions that calm the leg symptoms and build activity back gradually. Note whether discomfort stays in the buttock or travels below it, and report any sensory change or weakness.

Arrange medical assessment if pain persists, limits walking or sleep, or is accompanied by numbness or weakness. Seek emergency help for worsening weakness in both legs, numbness around the groin, or new difficulty controlling urination or bowel movements.
