Hives on legs: causes, relief, and red flags

Short answer
Hives on the legs are raised, itchy patches that can follow pressure, heat, illness, or exposure, but breathing trouble or facial swelling is an emergency.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Common local clues
- Socks, footwear, compression, shaving, heat, or sweating
- What to record
- Timing, exposures, medicines, illness, and photographs during a flare
- Emergency symptoms
- Facial or throat swelling, breathing trouble, swallowing difficulty, or faintness
Recognising the pattern
Leg hives may appear after socks, boots, compression, shaving products, sweating, cold air, exercise, food, medicine, or an infection. They can be pale in the centre, red around the edge, and intensely itchy. Their shape and position may change as one area settles and another develops. A mark that remains in one place and becomes warm, tender, scaly, blistered, or bruised needs assessment because it may be a different problem. [Hives](/conditions/urticaria/) and [Itchy Skin](/symptoms/itchy-skin/) explain the common terms.
Reducing irritation
Remove tight or rough clothing, cool the legs, and avoid hot showers while symptoms are active. Use gentle, fragrance-free cleansing and moisturising if the skin is dry, but stop any product that seems to worsen the reaction. Write down recent exercise, travel, illness, foods, medicines, insect exposure, and pressure from clothing. A pharmacist can advise whether oral antihistamine treatment is suitable for you. Arrange a review if episodes recur, spread repeatedly, disturb sleep, or do not improve. Do not self-start oral corticosteroids; a clinician decides when such treatment is appropriate.
Red flags to act on
Seek emergency care if hives occur with swelling of the face, lips, tongue, or throat; wheezing; shortness of breath; difficulty swallowing; faintness; or sudden collapse. Rapid progression after a medicine, food, or sting also warrants urgent help. Swelling beneath the skin may represent [Angioedema](/conditions/angioedema/). For non-emergency symptoms, seek clinical advice for painful or bruising lesions, fever, or marked [Skin Swelling](/symptoms/skin-swelling/).
Questions worth asking
Ask whether socks, footwear, compression, shaving, heat, or a medicine could be contributing. Ask how to choose an antihistamine safely, whether recurrent flares need testing, and whether the appearance fits hives rather than another [Skin Rash](/symptoms/skin-rash/). Confirm the emergency plan for facial or throat swelling and breathing symptoms.
