What should you know about hives on ears?

Short answer
Hives on the ears are usually temporary itchy welts, but swelling of the lips, tongue or throat needs urgent care.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

What it can mean
Hives, also called urticaria, are raised areas caused by substances released in the skin. They may be pink, red or close to your usual skin tone, and can change shape or appear in new spots. Heat, pressure from headphones or helmets, sweating, infection, medicines, foods or contact with a product can be triggers. A clinician may need to review the timing rather than rely on the ear location alone.
Hives can occur with deeper swelling, known as angioedema, around the ears, eyelids or lips. Itchy skin and a wider skin rash may accompany the welts. A mark that stays fixed, becomes painful, crusted or scaly deserves assessment because it may not be hives.
What to do now
Move away from a suspected trigger and remove irritating earrings, hair products, headphones or headwear if they seem relevant. Use a cool compress over the affected area and avoid scratching, hot showers and vigorous rubbing. Keep a note of new medicines, foods, illness, exercise and products used before the episode; photographs can help when the welts fade before an appointment.
A pharmacist or clinician can advise whether an oral antihistamine is suitable for you, including its timing and possible drowsiness. Do not put steroid creams, essential oils or other products into the ear canal unless prescribed. Recurrent or persistent episodes warrant a medical review.
When to seek care
Seek emergency help immediately for trouble breathing or swallowing, throat tightness, voice change, faintness, sudden weakness, or rapidly increasing swelling of the lips, tongue or face. These symptoms can signal a severe allergic reaction.
Arrange prompt medical advice if the ear is very painful, drainage develops, hearing changes, fever occurs, or the rash does not behave like fleeting welts. A clinician can also review hives that keep returning or interfere with sleep.
Questions for your doctor
Could pressure, heat, a medicine, infection or a contact product be triggering the ear welts? Does the swelling suggest angioedema? Which antihistamine is appropriate for my age, medicines and health conditions? What should I record if another episode appears?

Questions people ask
Can hives affect the ears only?
Yes. Hives may be limited to one area at first, although new welts can appear elsewhere as the episode changes.
Should I scratch hives on my ears?
No. Scratching can increase irritation; a cool compress and suitable antihistamine advice may help instead.
