What should you know about hip muscle strain?

Short answer

A hip muscle strain is an overstretched or torn muscle or tendon around the hip that can cause pain, tenderness, weakness, or pain with movement.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

At a glance

Often follows
A sudden stretch, forceful movement, or repeated loading
May cause
Local pain, tenderness, weakness, or painful movement
Helpful focus
Relative rest followed by gradual, comfortable movement

What it can mean

A strain may affect the muscles that lift the thigh, draw the leg inward, or extend the hip. It can follow a sudden change of direction, a forceful stretch, lifting, sport, or repeated activity. Pain may be felt in the groin, buttock, side of the hip, or upper thigh. Bruising, swelling, a spasm, or reduced strength can occur, although these findings are not present in every strain.

A clinician may assess walking, hip movement, muscle strength, and the exact tender area. Hip pain can also come from the joint, lower back, or another soft-tissue problem, so location and movement pattern matter.

What you can do

For the first day or two, reduce the activity that triggered the pain and use a wrapped cold pack for short periods if it feels helpful. Keep gentle, comfortable movement rather than staying completely still. As pain settles, gradually restore walking and light hip movement; avoid sprinting, deep stretching, or heavy resistance until strength and motion are returning.

A physiotherapist can guide a staged programme that may include gentle range-of-motion work, strengthening, and a gradual return to sport or work. Do not push through sharp pain. A pharmacist or clinician can help you choose pain relief safely, especially if you have other conditions or take regular medicines.

When to seek care

Arrange medical assessment if you cannot put weight on the leg, heard a snap, have marked weakness, increasing swelling or bruising, numbness, fever, or pain after a significant fall. Seek urgent help for a visibly deformed hip, a cold or pale leg, or severe pain that is rapidly worsening. If symptoms are not steadily improving, an examination can clarify whether this is a strain or another cause of hip pain.

Questions for your doctor

Ask which structure seems injured, what movements to avoid, when to begin strengthening, and what signs should prompt a review. Ask whether physiotherapy or imaging is appropriate for your examination findings.

Questions people ask

How is a hip muscle strain diagnosed?

Assessment usually combines your history with observation of walking, hip movement, strength, and tenderness. Further tests depend on the examination and injury pattern.

Can you exercise with a hip muscle strain?

Gentle comfortable movement is often reasonable, but painful or high-load activity should wait until movement and strength are improving.
