Mapping the discomfort

The bursa involved in trochanteric bursitis sits beside the greater trochanter, the firm prominence you can feel on the outside of the upper thigh. Pain may remain at that spot or spread down the outer thigh. Pressing the area, lying on that side, walking uphill, climbing stairs, or standing after sitting may reproduce it. Pain deep in the groin, pain mainly at the back of the hip, or pain travelling below the knee may suggest another or additional source. A clinician uses the full pattern rather than location alone.

What to try

Compare both sides gently to identify tenderness, without repeatedly pressing the painful area. Reduce direct pressure and change positions regularly. When sleeping on your side, try the less painful side with a pillow between your knees; a pillow under the knees may help when lying on your back. Keep walking within a comfortable range and avoid sudden increases in hills, stairs, or running. Physiotherapy can address hip strength and movement habits, while medical review can guide other treatments if symptoms persist.

Get prompt care for severe pain after a fall, inability to bear weight, a hot swollen hip, or fever. Seek urgent help for sudden leg weakness, spreading numbness, or loss of bladder or bowel control. Arrange a routine appointment when pain lasts, changes location, interferes with sleep, or fails to improve after reducing aggravating activities.
