Hiccups and throwing up blood: what to know

Blood in vomit may look bright red, dark brown, or like coffee grounds. It can come from bleeding in the food pipe, stomach, or upper intestine. Forceful vomiting can irritate or tear tissue, while gastritis and other digestive conditions can also bleed. Reflux may contribute to irritation, but hiccups do not explain away blood.

The amount and appearance can change, and even a small-looking episode can matter. Dizziness, weakness, pale or clammy skin, fast breathing, black stools, or fainting may suggest significant blood loss or shock.

Call emergency services or go to an emergency department now. Do not drive yourself if you feel faint or weak. Sit upright or lie on your side if you may vomit again, and have another person stay with you. Do not eat or drink while arranging care unless emergency clinicians tell you to do so. Avoid aspirin, ibuprofen, alcohol, and unapproved remedies, and do not take another person’s medicine.

Bring a list of medicines, especially blood thinners or anti-inflammatory drugs, and tell clinicians what the vomit looked like, when it started, and whether black stools or abdominal pain occurred.

Vomiting blood itself needs urgent assessment. Treat fainting, confusion, severe abdominal or chest pain, breathlessness, cold clammy skin, or ongoing vomiting as an emergency and call for immediate help. Do not wait for hiccups to stop or for symptoms to settle.

Be ready to describe the colour and amount of blood, previous vomiting, pain, dizziness, stool colour, alcohol use, ulcers or gastritis, and every prescription, over-the-counter medicine, and supplement. Ask when it is safe to eat, drink, and restart regular medicines after assessment.
