HFMD and sore throat: what to know

A sore throat can be an early presentation of hand, foot and mouth disease, especially when painful mouth ulcers appear soon afterwards. The throat discomfort may make drinking and eating difficult. HFMD is only one possible cause of throat pain, so the mouth, skin, fever pattern, and general condition should be considered together.

HFMD may produce small ulcers inside the cheeks, gums, tongue, or back of the mouth. These lesions can make swallowing sting and may be mistaken for an ordinary sore throat. Spots or small blisters on the hands and feet can support the diagnosis, although not everyone has the same visible pattern. Common cold, influenza, COVID-19, tonsillitis, strep throat, acid reflux, and allergic rhinitis can also cause throat symptoms, each with different clues and treatment needs.

Offer cool water, milk, or oral rehydration fluid in small, repeated sips. Soft foods such as yoghurt or soup that has cooled may be more comfortable; avoid citrus, spicy foods, and rough textures that irritate ulcers. Use pain or fever medicine suitable for the person’s age and health only as directed. Do not share drinks, cutlery, or toothbrushes. Handwashing after toileting, nappy changes, and wiping saliva helps reduce spread. Antibiotics do not treat a viral illness unless a clinician finds a separate bacterial infection.

Seek urgent help if the person is struggling to breathe, cannot swallow saliva, is drooling with distress, becomes confused, has a seizure, or is difficult to wake. Arrange prompt assessment when fluids are very limited, urine is much less frequent, pain is severe on one side, the voice changes markedly, or the neck becomes swollen. A clinician should review symptoms that are worsening, not improving, or accompanied by a spreading painful skin rash.

Ask whether the throat and mouth findings fit HFMD, how to protect hydration, and whether testing is needed for another cause of sore throat. Describe the onset of pain, ability to swallow, fever or rash, exposure to illness, and any breathing or voice change.
