HFMD without fever: what it can mean

Hand, foot and mouth disease can occur without fever, particularly when mouth sores or a mild rash are the main symptoms. The absence of a high temperature does not rule it out, and a fever is not required for the infection to spread. Look at the full pattern, including mouth discomfort, skin changes, energy, and drinking.

HFMD commonly causes painful spots or ulcers inside the mouth and a rash on the palms, soles, fingers, or around the nappy area. Some people have only a few skin lesions or mild mouth symptoms. A child may still be contagious while feeling reasonably well. Other conditions, including chickenpox, cellulitis, or a common cold, can cause overlapping symptoms, so a clinician may need to examine the mouth and skin if the pattern is unclear.

Give cool fluids often and choose soft foods if chewing or swallowing hurts. Avoid foods that sting open mouth sores. Keep nails short and discourage scratching, which can damage the skin. Wash hands after toileting, nappy changes, and contact with saliva; do not share cups, cutlery, towels, or toothbrushes. Clean commonly touched objects and keep the child home while they are unwell, following local school or nursery guidance. Do not use antibiotics unless a clinician identifies a bacterial infection.

Arrange medical advice if the child cannot drink comfortably, has fewer wet nappies or toilet visits, or becomes unusually tired. Seek urgent help for breathing difficulty, a seizure, confusion, a stiff neck, or trouble waking. Have a clinician check skin that becomes increasingly red, hot, swollen, painful, or produces fluid. Review is also sensible when sores are worsening, symptoms are not improving, or the child has a weakened immune system.

Ask whether the mouth and skin findings fit HFMD without fever, how long to avoid close contact, and what hydration signs to monitor. Tell the clinician when the spots began, whether new lesions are appearing, how much the child drinks, and whether urine output has changed.
