How long can HFMD fever last?

HFMD fever often settles within a few days, but the child’s hydration and overall condition matter more than the exact duration. Mouth ulcers and a rash can remain after the temperature improves, and appetite may take longer to return. A changing pattern, rather than the calendar alone, helps guide care.

Hand, foot and mouth disease can begin with fever, tiredness, reduced appetite, or a sore throat. Mouth sores and spots on the hands, feet, or other areas may appear around the same time or later. Fever that is easing while the child drinks, urinates, and becomes more alert is generally reassuring. Fever that continues without improvement, returns after settling, or occurs with a new concerning symptom deserves clinical review. Fever can also have causes unrelated to HFMD, such as bronchiolitis or another infection.

Check the child regularly rather than repeatedly chasing a number on a thermometer. Offer small, frequent drinks and choose cool, soft foods if the mouth is sore. Rest is helpful, while light clothing and a comfortable room can prevent overheating. Use a child-suitable fever medicine only as directed on its label or by a clinician; do not alternate medicines unless advised. Keep towels, cups, and utensils separate, wash hands carefully, and clean surfaces touched by saliva or stool.

Contact a clinician if the fever is not easing, returns after a period of improvement, or the child remains markedly unwell. Ask for same-day advice when drinking is poor, vomiting prevents fluids, the mouth is too painful for swallowing, or wet nappies or urination decrease. Seek urgent help for breathing problems, a seizure, a stiff neck, confusion, severe weakness, or difficulty waking. A spreading red, hot, swollen, or draining area of skin also needs assessment.

Ask what course is expected for this child, how to judge hydration, and which symptoms would change the urgency. Bring the temperature pattern, medicine times, fluid intake, urine changes, rash progression, and any known contact with HFMD to the discussion.
