HFM rash on legs: what to know

An HFM rash on the legs should be judged by its shape, spread, sensation, and whole-body symptoms rather than location alone. Look for dry plaques, small bumps, blisters, crusting, rings, or colour changes. Note if both legs are affected, if socks or shaving preceded it, and whether warmth, swelling, pain, or fever is present.

Friction, sweat, shaving, and contact with plants or products can irritate the legs. Follicles may become inflamed after hair removal, while fungal infections can form scaly borders. Itchy patches may also occur with eczema or psoriasis. A red, hot, swollen, painful area—especially with fever—needs medical assessment because infection can worsen quickly. Rinse off possible irritants, use gentle cleansing, and moisturise dry intact skin. Avoid shaving, tight clothing, vigorous rubbing, and fragranced products while the area is unsettled. Do not scratch open areas or share towels. If the rash followed a medicine, do not stop an essential treatment without medical advice; contact the prescriber and describe the timing. Seek urgent care for rapidly spreading redness, severe pain, marked swelling, fever with a hot tender leg, pus, red streaks, purple spots, or feeling faint. Emergency help is needed for breathing difficulty or swelling of the face, lips, or tongue. Arrange a review for a recurrent rash, a nonhealing sore, or symptoms that continue despite removing an irritant.

Ask whether the rash follows follicles, a contact area, or a ring; whether testing is needed; and which cream is appropriate for the exact site. Mention shaving, travel, new clothing, pets, medicines, and anyone else with similar symptoms.
