HFM rash: what to know

An HFM rash is not a diagnosis by itself; its distribution, texture, timing, and associated symptoms guide next steps. It may appear as flat colour change, raised patches, flaky skin, welts, or fluid-filled spots. Record whether it is itchy, tender, warm, or linked with fever, sore throat, or feeling unwell.

Rashes that follow a new soap, fabric, adhesive, or cosmetic may reflect irritation or allergy. Sweating and friction can worsen heat-related inflammation, while fungal or mite infections may have characteristic borders or nighttime itch. A medicine-related eruption can appear after starting or changing a treatment. These possibilities overlap, so the whole history matters. Keep the area cool and dry, wear loose breathable clothing, and cleanse without scrubbing. Stop nonessential new products and avoid sharing towels, clothing, or personal items until an infectious cause has been excluded. Do not burst blisters or apply several active creams at once. Take clear photographs in consistent light and note changes after each product or medicine.

Get urgent care for breathing difficulty, swelling of the lips or tongue, faintness, extensive blistering, skin peeling, severe pain, or a rash involving the eyes. Seek prompt advice if fever accompanies a rapidly spreading rash, if there is pus or red streaking, or if the eruption began soon after a new medicine. A persistent rash deserves assessment even when discomfort is mild.

Ask what pattern the clinician sees, whether testing is needed, which products should be avoided, and how long to try the recommended treatment. Mention pregnancy, allergies, immune conditions, and every recent medicine, supplement, or topical product.
