What should you know about hfm diaper rash?

Hand, foot, and mouth illness can bring fever, mouth sores, and spots or blisters on the hands and feet; some children also develop spots around the bottom or nappy area. A red rash limited to areas exposed to urine or stool is often irritant dermatitis. Bright redness in the folds with small spots beyond the edge can suggest yeast, while blisters, crusting, pus, or rapidly spreading redness need a clinician’s assessment.

The nappy rash pattern alone cannot confirm hand, foot, and mouth illness. The child’s fever, mouth symptoms, hydration, exposure history, and general behaviour help guide the diagnosis.

Change nappies frequently, rinse with lukewarm water or use a fragrance-free cleanser, and pat rather than rub. Allow short periods of air exposure when practical, then apply a thick protective barrier recommended for babies. Avoid fragranced wipes, talc, scrubbing, and steroid or antifungal creams unless a clinician advises them. Emollient therapy can support a damaged skin barrier, while topical antifungal treatment is used only when yeast is suspected or confirmed. Encourage fluids and offer soft, cool foods if mouth sores make eating uncomfortable.

Seek prompt medical advice for a young infant with fever, a rash that is blistered or infected-looking, worsening pain, or skin that is not improving with gentle care. Get urgent help if the child is unusually sleepy, struggling to breathe, unable to drink, passing very little urine, or showing signs of dehydration such as a dry mouth and no tears. Medical review is also needed if fever persists or the rash spreads quickly.
