What should you know about heterotopic pregnancy twins?

Heterotopic pregnancy is a form of multiple pregnancy involving different implantation sites. One embryo develops in the uterus, while another implants most often in a fallopian tube. The uterine pregnancy can make the condition harder to recognise because an early scan may show an intrauterine pregnancy without excluding a second pregnancy elsewhere.

Possible symptoms include one-sided pelvic or abdominal pain, vaginal bleeding, shoulder-tip pain, dizziness, fainting, or pain that worsens with movement. Symptoms vary, and their absence does not rule out the condition. Ultrasound, examination, and pregnancy-related blood tests are interpreted together.

Contact your maternity team or an emergency service immediately if heterotopic pregnancy is suspected or if you have significant pain, bleeding, weakness, or faintness. Tell them how far along the pregnancy may be and whether fertility treatment was used, because that context can affect the assessment.

Treatment depends on the location of the ectopic pregnancy, bleeding, stability, and the status of the uterine pregnancy. The ectopic pregnancy may require surgery or medication, while the intrauterine pregnancy is monitored separately. If surgery is needed, a Caesarean Section may later be discussed for obstetric reasons, but it is not the treatment for the ectopic pregnancy itself.

Go to emergency care for sudden or severe abdominal pain, shoulder-tip pain, heavy bleeding, collapse, marked dizziness, pale or clammy skin, or difficulty breathing. These may indicate internal bleeding and need immediate evaluation.

Even mild one-sided pain or spotting deserves same-day advice when pregnancy is possible. Do not drive yourself if you feel faint. A Growth Scan is used later to monitor fetal development, but it cannot replace urgent assessment of acute pain or bleeding.
