Hepatitis B immunoglobulin for a newborn

Immunoglobulin contains antibodies that can act quickly. It is different from the hepatitis B vaccine, which teaches the baby’s immune system to make its own longer-lasting response. When clinicians recommend both, they serve different purposes and may be given during the same early-care period.

Make sure the maternity team knows the birth parent’s hepatitis B status and bring any antenatal test results if available. Ask staff to record the immunoglobulin and vaccine doses in the baby’s health record. Keep follow-up appointments so the care team can confirm the remaining vaccine schedule and decide whether testing is needed later.

Do not delay advice while waiting for symptoms; a baby exposed to hepatitis B may look well. For later concerns such as unusual sleepiness or feeding difficulty, contact the child’s clinician.

Tell the neonatal team immediately about breathing difficulty, facial swelling, collapse, or a rapidly spreading rash after an injection. Outside the hospital, seek urgent help if the baby is difficult to wake, feeds poorly, has repeated vomiting, or shows yellow skin with worsening illness. These signs require prompt assessment rather than home observation.

Ask why immunoglobulin is advised, when the vaccine is due, how the doses will be documented, whether follow-up blood testing is planned, and whom to contact after discharge.
