Hepatitis B vaccination for newborns: pros and cons

Hepatitis B can infect the liver and may cause no obvious symptoms at first. Newborns can be exposed around delivery if the birth parent has the virus, including when the infection is not yet known. Vaccination gives the baby protection early in life. The usual trade-offs are a needle, temporary tenderness at the injection site, fussiness or a mild fever. Serious reactions are uncommon, but the maternity or paediatric team can explain what to watch for based on the baby's health.

Tell the maternity team the birth parent's hepatitis B test result, if known, and share any previous medical information. Ask when the first vaccine dose is due and how later doses will be recorded. If the birth parent has hepatitis B, the baby may need additional protection and carefully timed follow-up arranged by the clinical team. Keep the vaccination record and mention prematurity, illness or a previous reaction before each dose. A paediatrician may recommend blood testing later to check the response.

Contact the baby's healthcare team urgently for breathing difficulty, swelling of the face or throat, unusual limpness, or a rapidly spreading rash after vaccination. A mild fever or soreness can occur, but call for advice if the baby is difficult to wake, feeds poorly, has repeated vomiting, or seems increasingly unwell. If the birth parent's infection status is positive or uncertain, seek clinical advice promptly rather than delaying the newborn protection plan.

Ask what protection is needed at birth, which doses come next, how reactions should be managed, and whether follow-up blood tests are planned. Clarify who will update the record if care moves between the maternity unit and a community clinic.
